Refer a Child for a Mentor

Do you know a young person in need of a mentor?

Eligibility Requirements:

For PALS, the community-based program:

  • The child is between the age of 8 and 13 at time of referral
  • The child wants a mentor and is willing to participate in the program, and does not feel forced to participate
  • The child’s needs are such that they can be reasonably met by a volunteer, nonprofessional mentor
  • The parent/family is willing and able to play a supportive and cooperative role in helping to support and sustain a match relationship

For LDL, the school-based program:

  • The child is between 5 and 17 at time of referral
  • The child wants a mentor and is willing to participate in the program, and does not feel forced to participate
  • The child’s needs are such that they can be reasonably met by a volunteer, nonprofessional mentor
  • The parent/family is willing and able to play a supportive and cooperative role in helping to support and sustain a match relationship

Whether you are a service provider or parent, you can start the referral process today! Simply download the referral form (2014) and complete it.

WCP also asks that parents/guardians complete a Permission for Release of Information (2014) form. The purpose of the release form is to assist WCP in determining a child’s eligibility and priority for placement in the program. It is also helpful in making a successful match between the child and adult volunteer and allows us to provide ongoing support to the partnership. After all the information is completed, mail your referral paperwork to WCP at:

We will contact you when we have received your referral and added the child you referred to our waiting list. Please be aware that, because of the geographic breadth of our service area and our customized matching process, matching times can vary significantly and can take a long time.
